SMI Case Manager Job Summary: The Case Manager, using discretion and independent judgement, is responsible for the delivery of quality patient care to members via the coordination of a member’s recovery team in securing and managing general medical and behavioral health services that aid in the member’s recovery from mental illness and/or addiction.

  •  Participates in the assessment, evaluation and planning of patient care.
  • Develops initial individualized service plans for members.
  • Provides community and home based services to enhance and achieve treatment goals.
  • Develops, facilitates and coordinates CFT/ART meetings, staffing’s, case conferences and other meetings as appropriate.
  • Provides referrals to outside providers and assists in securing a variety of social services, including, but not limited to, relapse prevention services, medical care, food stamps, general assistance and child-care.
  • Maintains a minimum number of documented client contact hours per week (22 hpw).
  • Maintains appropriate documentation (charting) and documents accurately and timely in accordance with agency and funding source requirements.
  • Communicates with member (or member’s family) to coordinate consent to care, releases and/or referrals for provider care which includes behavioral care, general medical care, community resources, educational, social, judicial and other State agencies.
  • Coordinates transportation of members to and from appointments when appropriate.
  • Consults with treatment staff to coordinate effectiveness of services contained in the members individual service plans and make adjustments where needed.
  • Provides outreach and follow-up on crisis contacts and missed appointments.
  • Meets with service providers to review utilization and monitor member progress towards program goals.
  • Provides member advocacy and education as needed, to ensure delivery of required services.
  • Develops transitional service plan (including discharge planning) with members, prior to reentry into the community.
  • Performs other related duties in accordance with agency growth and changes.
  • Preserves the human dignity and the confidentiality of each patient.
  •  All clinical staff will participate in the rotation of “on-call” duties by the carrying of the company on-call cell phone. This duty includes overnight, weekends and holidays.

Qualification Requirements


  • Master’s degree in a behavioral Health related field, OR,
  • Bachelor’s degree in a behavioral Health related field with one year behavioral health experience as a case manager/care coordinator, OR,
  • Is an Arizona Licensed Registered Nurse with one year behavioral health experience


  •  Demonstrates quality written documentation that reflects correct spelling, grammar, appropriate use of behavioral health acronyms and appropriate use of therapeutic terminology.
  •  Demonstrates team work and shared leadership thru positive and supportive communications.
  •  Excellent technical capacity including detail orientation.
  •  Carries cell phone and is able to be reached during work hours; returns emails/phone calls within a 24 hour turnaround time frame.
  •  Collaborates (consults) with his/her supervisor or clinical consultant when support is needed.
  •  Flexible with excellent time management.
  •  Knowledge of the standard principles, methods, practices, techniques, tools, and equipment used.
  •  EMR record keeping of required and billable services.
  •  Ability to organize, plan and communicate effectively and professionally.
  •  Exhibit professional ethics and boundaries.
  •  Must be able to work in a diverse, multi-cultural environment and be sensitive to the service population’s cultural and socioeconomic characteristics.
  •  The ability to interact effectively with other team members in a team based approach to care management. 

Physical Requirements

  •  Clinic/Office environment with frequent sitting, standing, writing and computer usage. 
  •  Must be able to climb stairs, with frequent standing and moving throughout facility or in the alternative, perform these physical requirements with or without reasonable accommodations.
  •  Frequent to moderate physical effort (lift/carry up to 25 lbs.).
  •  Frequent to moderate reaching, bending, stooping, kneeling or crouching.
  •  Must be able to physically demonstrate competency in the operation of an agency transport vehicle (if employed as a driver).
  •  Must be able to physically demonstrate competency in the operation of one’s own vehicle (if job includes driving client
